| Findings: The complaint alleges that a teacher working with preschool-aged children at the center grabbed a 3-year-old child by the shirt and yelled at the child. It was reported that the child did not want to attend "school" because the teacher was mean. According to the reporter, the child also reenacted the incident for an adult, demonstrating how the teacher grabbed their shirt. The center director and a staff member who worked alongside the teacher were interviewed. Both stated they had never witnessed the teacher use physical discipline with children; however, they described her tone as harsh and stern. The director reported that she was aware of the incident and had received numerous complaints from parents indicating that their children referred to the teacher as "mean." The director stated that she addressed the incident with the teacher and discussed her tone, discipline approach, and age-appropriate expectations for working with preschool-aged children. However, the director indicated that she did not believe the teacher was a good fit. As a result, the teacher has not returned to the center since 5/8/26, and the center is actively interviewing candidates. See remarks. | |||

| Findings: The caller reports that the outside area is unkept with the barrier showing where mulch should be and piles of leaves where animals could be hiding. At the time of the inspection, the black tarp was visible on the playground underneath the mulch which can be a trip hazard to the children. It was also noted that the mulch on the playground needed to be replenished. The specialist did not observe piles of leaves on the playground during the inspection. The director stated that the mulch has been ordered and will be here this week. The center is reminded that the activity area shall be free from potential hazards to child health or safety. | Findings: The complaint alleges that aides are left alone with 15 to 20 children. At the time of the inspection, the specialist did not observe the aides supervising 15 to 20 children alone. There was no evidence to verify that this has happened prior to today. During the inspection the specialist observed the aide in the preschool classroom 1C leave another staff member alone in the classroom with 13 children so she could grab the children's lunch across the hall in classroom 1A. The specialist also observed the staff member who was substituting in the infant room leave the classroom with one infant, while the other staff member in the classroom was left alone to supervise 4 infants. | Findings: Upon arrival in the infant room, the specialist observed an infant sleeping in a bouncy seat and informed the staff in the classroom that the infant must be placed in an approved rest furnishing immediately. Approximately an hour later the specialist returned to the infant room and observed 2 different infants resting in the bouncy seats. The specialist informed the director and the director spoke with the staff having them to remove the infants from the bouncy seats into the cribs. The director stated that the staff uses the bouncy seats to rock the infants to sleep and wants to ensure that the infants are fully asleep before placing them inside of the cribs. The specialist reminded the center that children should be immediately moved to an approved sleep source once they have fallen asleep. | Findings: Complaint states that there are two new adults working with the children in the preschool room. Each of these new staff are/have been alone with the children. Findings: Center states that 2 aides were substituting for the lead teacher in the preschool room last week and this week. Specialist does not have a criminal background checks or releases for this center for them. These two staff persons do have criminal background checks on file at the office of child care for other centers. Specialist did receive a notarized release on site for one of the aide substitutes. The employee sign in sheet showed two male person's had signed in to work. Specialist does have a criminal background check for one of these staff persons but not a notarized release. Specialist does not have either a criminal background check or notarized release for the other male staff person. When specialist arrived today all the staff were staff who are cleared. However it was mentioned that one of the aides who substitutes and is not cleared was present this morning with the children. Center is reminded that no one may be with the children who is not cleared by specialist. | |||
