Date Type Regulations Status
2026-05-01 Complaint 13A.16.08.01B(1) Corrected
Findings: The Office of Child Care received a report concerning an infant who was not fed properly. The center uses the ProCare app to record feedings. One staff in the infant room recorded that the infant was given 3 bottles and a pouch of food throughout the day. At pick up time, the parent was given 2 empty bottles and 1 full bottle. The parent inquired about this, concerned that her child may have gotten another child's bottle. One staff in the infant room indicated that the 11am feeding was entered as a bottle mistakenly, and that the child was given food. The camera footage was reviewed by the director and there was no feeding at 11am. The one staff was adamant that food was given. The admin staff are not sure if the one staff believes she actually gave the child food, or if she forgot and then recorded it anyway to cover her mistake. The director terminated the staff. The director met with all of the infant and toddler staff to discuss infant feeding. There are activity plans posted in the classroom and a dry erase chart that shows each child's last feeding and next feeding. There are labeled bins in the refrigerator and on a shelf in the classroom for the children's food.

2024-04-15 Complaint 13A.16.07.03A(1) Open
Findings: The complaint alleges a three-year-old boy with autism had a cloth face mask placed on him covering his mouth and nose because he was spitting. While he was wearing the face covering, it is alleged the teacher was holding the child's arms down while he was standing up throwing himself back and forth. The teacher allegedly let the child go when the parent arrived in the classroom around 515 PM on April 11th. It is further alleged that the center states it is their policy to put face coverings on children who spit because there are a lot of germs in their mouths. At today's inspection, the director and staff member 1 involved in the incident were interviewed and a copy of the discipline policy was given to the specialist. The center has a spitting policy in their handbook that they follow and the director states after several verbal warnings and explanations regarding spitting the children are given a mask for about 3 to 5 minutes. The director states that the 3-year-old child is physically able to remove the mask from his face. Staff member 1 denied holding the child's arms down on April 11th. She also states that redirection is used with the children. Please see the remarks.
