| Findings: At the time a complaint inspection was being conducted, it was noted that there were a couple times this week when there were too many two year old children with one staff. One May 9th, it was documented that there were 8 two year old children with one staff person at 9:00 AM and the same staff person had 9 two year old children on May 9th around the same time. The second staff person begins work at 9:30 AM and that is when the ratio is fixed. The center owner stated she was unaware the classroom had been out of ratio and stated she will put another staff person in the room earlier to ensure the ratios are met. | |||

| Findings: The complaint alleges there are 12 children enrolled in the preschool classroom of which the capacity is 8 and 14 children enrolled in the two year old room of which the capacity is 12. It is reported there are 2 staff in the preschool room and 3 staff in the two year old room. The complainant states this is every day and the food program reports will confirm the number of children. At the time of the inspection, there were 10 children in the preschool room. The owner stated she though the capacity of that room was 10. It was changed when the school age room expanded to 30 from 28. There are 12 flower pots on the classroom door with children's names and there are 12 name on the food program attendance sheet. The attendance sheets for this room showed 11, 12 and 13 children on dates in March and April at times around 12:00 PM. The preschool room's capacity is 8. The owner moved 2 children to the other preschool room to bring the classroom back to its assigned capacity. The two year old room had 9 present and the attendance records were reviewed. There were never more than 12 children present. There are 13 names though on the food program attendance. | |||

| Findings: The center self reported an incident that occurred on 1/24/2024. The school age teacher returned to the center after the morning van runs and parked the van in the Salvation Army parking lot across the street. He returned to the building and was cleaning the school age room. Approximately 10-15 minutes later, a woman arrived at the center with an 8 year old child she said was about to cross the street after exiting the center van. The child said she had fallen asleep with her feet on the van chair. The school age teacher had marked the child on the attendance sheet as being dropped off at school. The child resembles another child in care and he may have mistakenly marked her off. He failed to do an in the van check and only checked in the vans windows while walking around it to ensure the van was empty. The school age teacher says he did not see the child on the van when he checked. The owner stated the vans are newer and have higher backed seats which requires a more detailed check of the seats. The child's mother was notified and picked her up at the center around 12:30 PM. The school age teacher has been placed on administrative leave during the investigation. | |||
