| Findings: A complaint was received that in part alleged the site director has had her 2 year old child at the site with her. This site is only approved for preschool and school age children that attend the school. Staff were interviewed, including the site director, and it was disclosed that the site director on occasion has had her child at the center in the morning and in the afternoon due to childcare issues. The site director stated she was informed by the area manager that she could not bring her child to the center and she has complied. The area manager and the regional manager coincidently came to the site to perform a regular observation of the center while the LS was conducting the inspection. The area manager confirmed that on a pop up visit she made to the center the site director's child was present. The area manager sent the site director home that day and explained she could not have her child at the center at any time. | |||

| Findings: A complaint was received that alleged that the staff at the site are not supervising the children in the cafeteria. It further alleged children have been observed to be running around, jumping on tables, hitting each other, fights breaking out and staff not doing anything about it. The reporter alleges that children attending the program and parents of children attending the program have complained about the behaviors and lack of adult supervision. Upon arrival the children were scattered between the gym and the cafeteria. The site director explained that the children are divided into 3 groups based on age. They alternate between the gym and cafeteria. There were 16 children with a teacher and an aide in the gym and 8 children in the cafeteria with a teacher and the site director. During the inspection, the children were observed climbing on the mats that were stacked in the gym even after the staff told them that was not permitted. There were toys scattered on the floor in the cafeteria. At one point 8 children were left alone in the cafeteria while the teacher left to answer the door for a parent. The site director denies that she spit on the child and that she instead wiped the spit in the child's mouth on the child's shirt. Licensing specialist interviewed the other staff present. Two of the staff stated before the site director took the child into the dark kitchen they heard her tell the child "kick me and I will kick you", all of the staff report that the site director will take another child into the kitchen with the lights off to scare the child by telling the child that a one eyed monster is going to get them.
