She has only had 1 DTP, 1 Polio and 1 Hep B (of which she needs 3 of each). She has 1 Prevnar and 1 Hib (of which she needs 2 of each). On one of the Health Inventory Part II forms, the father signed for the religious exemption, but that form does not contain the physician's signature. On another Health Inventory Part II, the physician completed and signed it, but it does not contain anything regarding a religious exemption. It may be that the parent has not provided her immunization from her 6 month old check up to the Provider. Please follow up with the parents of ED regarding her immunizations and notify OCC of the outcome. | Findings: There was a baby sleeping in a separate room from where the provider and other 4 children were working on an art project. The provider could hear the baby, if needed, but not see him. The provider has a video monitor but wasn't using it. The provider did obtain the video monitor during the inspection. Please be sure to always keep sleeping or resting infants in the same room with you or use the monitoring video device in the future. The baby woke up within minutes and joined the group. This non-compliance was corrected on site when the baby woke up. The provider stated that she decided to work on a Christmas art project when the baby fell asleep.
