Findings: There is one drawer in the kitchen which is not latched that contains adult scissors, pliers and batteries and other potentially hazardous items. Provider should latch this drawer or remove and relocate these items to make them inaccessible to children. Please send photo of latched cabinet or send written correction stating items have been relocated.

Findings: Licensing Specialist observed Lysol spray and a bottle of bleach on the bathroom floor today during inspection. These items are potentially hazardous to children and should be relocated immediately. Provider relocated the items while Licensing Specialist was present, therefore, this has been corrected.

Findings: Provider's diaper changing mat has a small tear in the vinyl which could allow the pad underneath to become wet or soiled, which is not sanitary. Provider should replace the diaper changing mat and send copy of receipt or photo to Licensing Specialist within 30 days of inspection.
