Noncompliance Area: During the renewal inspection conducted 4/15/26, upon request there was no emergency drill log available for review to verify the facility conducts annual emergency drills.

Correction Required: Emergency drills shall be conducted annually. Annual emergency drills shall be documented and on file at the facility.

Description: Training regarding plan

Noncompliance Area: During the renewal inspection conducted 4/15/26, the emergency plan reviewed for staff #1, #2, #3, #4, #5, #6, #7, and #8 was not annually updated. Last dated 3/3/25. The record reviewed for staff #9, #10 and #11 did not include initial emergency plan training.

Correction Required: The operator shall assure that each facility person receives training regarding the emergency plan at the time of initial employment, on an annual basis and at the time of each plan update. The operator shall document the date of each training and the names of all facility persons who received the training and kept on file at the facility.

Description: Building Surfaces

Noncompliance Area: During the renewal inspection conducted 4/15/26, water stains were observed in the ceiling in room 4-5 classroom.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.

Description: No peeling paint or plaster

Noncompliance Area: During the renewal inspection conducted 4/15/26, chip paint was observed in room 1 on the light blue wall near the cots and outlet. In room 4-5 chip paint was observed on the light orange wall.

Correction Required: Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility.

Description: Building Surfaces/Clean, good repair, proper size

Noncompliance Area: During the facility inspection there was a large unmounted shelf on the outdoor play space.. The shelf had various items on each tier, i.e. water hose, wooden crate at the top, miscellaneous objects in a large green bag.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Furniture must be durable, safe, easily cleaned and appropriate for the child's size, age and special needs.

Description: Building Surfaces/No peeling paint or plaster

Noncompliance Area: At the time of inspection, there was damaged ceiling tiles in the facility classroom which could pose a mole hazard. Accessible on the facility outdoor playground; there was a large piece of wood (that a child could reach), on the outside of a window sill.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility.

Noncompliance Area: During time of inspection, there was a blue chair in room 4-5 that was in need of repair or replacement. In addition, there was a Blue & Gray Castle slide plastic playset with steps that were cracked and in need of replacement or disposal. There was also a large brown & blue plastic bridge with flooring that was cracked and beyond repair which could pose a hazard to children that should be removed and/or replaced.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. Furniture must be durable, safe, easily cleaned and appropriate for the child's size, age and special needs.

Description: Age appropriate screenings complete/Exemption documentation from parent/guardian

Noncompliance Area: At time of inspection, Child #1 did not have current immunization for influenza documented in their record or exemption letter signed by parent.

Correction Required: A health report shall include a statement that age-appropriate screenings recommended by the American Academy of Pediatrics were conducted since the time of the previous health report required by this section. Exemption from immunization for religious belief or strong personal objection equated to a religious belief shall be documented by a written, signed and dated statement from the child's parent or guardian. The statement shall be kept in the child's record.

Description: Age and Training/Exp, educ., training at facility

Noncompliance Area: Staff person #1 did not have required current first aid/CPR training documented in her record as required per regulations.

Correction Required: Competence is the completion of training by a professional in the field of first-aid and cardiopulmonary resuscitation (CPR). All staff persons shall renew their certification in pediatric first aid and pediatric cardiopulmonary resuscitation (CPR) on or before the expiration of the most current certification. A facility person's record shall include verification of child care experience, education and training following the outset of service at the facility.

Noncompliance Area: Staff person #2 did not have valid required child abuse clearance documented in their record.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). SAFETY PLAN: The operator is required to submit to the Children and Youth Agency a plan of supervision or alternate arrangement as required by the Child Protective Services Law. LACKING REQUIRED HIRING DOCUMENTS: Facility Person # 2 may not work in a child care position at the facility. Facility Person #2- may not work in a child care position at the facility.

Noncompliance Area: During time of in-person visit, there was a blue children's chair in room # 4-5 that was torn on the arm with exposed white cushioning that is in need of repair due to the exposed cushioning could pose a safety concern for the children.

Correction Required: Furniture must be durable, safe, easily cleaned and appropriate for the child's size, age and special needs.
