Findings: An email stated that a child had been bitten in the face and also had a bruise on her face that could not be explained by staff. The parent of the bitten child was later told there was another biting incident but that was not reported until two days later. During the interview the center stated that they talked to the parent the day of the incident but did not have an explanation to the incident or injury or write an incident report. The center did report that they gave the parent another incident report from another incident that week. Center was reminded to notify parents of injuries to children.

Findings: An email sent in stated that a child has been bitten in the face and also had a bruise on her face that could not be explained by staff. It was also reported that a child was sent home in Pampers diapers rather than the pull ups provided to the center by the parents. During an interview with the director it was stated that the staff in that classroom did not witness the child's injury as one staff was changing diapers and the other staff was with the children did not witness it. It was also reported that one staff mislabeled the child's diapers that lead the child to be put in the wrong diaper. One staff has been terminated by the center and the other staff is no longer in that classroom. The center director has assigned new staff to the classroom. The center confirmed that the child was injured and there were two staff present that at least one of the staff should have known about the injury.

Findings: An email stated that the center told parents that there would be 3 to 4 teachers in the classroom but reportedly there has only been 1 to 2 teachers in the classroom. After reviewing the children's attendance during the time of complaint the center was in compliance with qualified staff and in ratio with 2 staff. However at the time of the complaint inspection the center did not have a qualified staff in the classroom due to changing the staff in the classroom. For correction please send an updated staffing pattern showing that there is a qualified teacher in the classroom.
