| Findings: A substitute has been used since a staff person left in February. The OCC did not receive a notarized Release of Information form or criminal background check results for this substitute. Therefore, this substitute was not cleared to be able to work with the children. There were 2 teachers working for whom the OCC never received a copy of a criminal background check. It was reported that the center received their copy of the criminal background check. The Licensing Specialist received the notarized Release of Information form during the inspection for the substitute. The center stated that the substitute will complete their criminal background check on June 2, 2025. During the inspection the Licensing Specialist received a 365 form to submit to CJIS to receive the OCC's copy of the criminal background checks for one of the teachers. The center stated that they would submit the 365 form for the other teacher on June 2, 2025. The director was reminded to wait for clearance verification from the OCC prior to allowing staff to work. | |||
