| Findings: Supervision Concerns. This provider self-reported that a child had escaped from the play yard through a hole in the fence. During investigation, the Health department verified the child was able to leave the facility through a hole in the fence and was found a few minutes later in a parking lot of a neighboring business. Parents were notified. Resolution: The hole in the fence was fixed the same day. The Health Department confirmed the repair and it appeared sturdy. Provider is working with IdahoSTARS to have staff take an Active Supervision training. | |||

| Findings: 20-00218/Supervision Concerns. Two children were able to inappropriately touch each other as there was a lack of supervision at the child care facility. Resolution: The provider has implemented several trainings and had a team meeting with all the team members about what happened and when and how to properly report it and to whom. The staff also watched the "care enough to report" video the next day. | |||
