| Findings: Health & Safety Concerns. There were concerns that the illness policy at this facility was not being followed. During the investigation is was confirmed that families had not been informed about an RSV outbreak, children with fevers were not being sent home, and staff was being directed to work while sick. Resolution: Due to numerous substantiated incidents and lack of ability to remain in compliance, this provider's Daycare License was Summarily Suspended. | |||

| Findings: Supervision Concerns. There were concerns that the facility is frequently over child/staff ratios and that the facility was not up to health & safety standards. During the investigation, the inspector confirmed that this facility was significantly out of child/staff ratios. The cleanliness of the facility was noted to be relatively clean and did not pose any concern to the inspector. Resolution: A staff member arrived on scene to put the facility back into ratio compliance. The Department issued a Notice of Concern Letter in regard to the supervision concerns that were substantiated. The health department performed a follow-up investigation and no new violations noted. | |||
