The school reviewed the list of the children in Pre-K3 and Pre-K4 who attend the before and after school care program. There are only 7 children in our preschool program who attend the extended care service, and all seven preschool children have their emergency contact forms. However, the school age children would need to complete the required form from DHS for the extended care program. The school has used a different format for the extended care registration.
