Noncompliance Area: At the time of the inspection, child #8 was observed sleeping in a crib with a bottle in their mouth.

Description: Restraints prohibited

Noncompliance Area: On 5/21/25, an unannounced visit was conducted. After interviews with staff, it was determined that child #1 is being placed in a highchair for up to ten minutes at a time, about 3-10 times per day for behavioral purposes. During this time that the child is in the highchair, his movement is restricted.

Correction Required: A facility person may not restrain a child by using bonds, ties or straps to restrict a child's movement or by enclosing the child in a confined space, closet or locked room. The prohibition against restraining a child does not apply to the use of adaptive equipment prescribed for a child with special needs. Tiered LIS: 1.) Noncompliance Area: At the time of the inspection, there was no parental written consent for the Zyrtec present at the facility for child #8.

Noncompliance Area: At the time of the inspection, the health report dated 9/9/24 on file for staff person #18 did not include a physical examination, as required.

Noncompliance Area: At the time of the inspection, the health report dated 8/26/24 on file for staff person #16 answered "No" to the person's suitability to provide childcare with no additional follow up information, as required. The health report dated 9/9/24 on file for staff person #18 did not include an examination for communicable diseases and the results of that examination. It also did not include the physician's or CRNP's assessment of the person's suitability to provide childcare.

Correction Required: An adult health assessment must include an examination for communicable diseases and the results of that examination. An adult health assessment must include the physician's or CRNP's assessment of the person's suitability to provide child care.

Noncompliance Area: At the time of the inspection, staff person #2 only had one written, nonfamily references from individuals attesting to the person's suitability to serve as a facility person on file.

Description: Comply with CPSL/CPSL information

Noncompliance Area: At the time of the inspection, staff person #8 had a PA State Police clearance dated 1/18/23. This clearance read "Request still Pending" and did not contain a dissemination date making this an incomplete clearance. Staff person #10 did not have a current NSOR certificate on file. This is evidenced by the NSOR certificate on file dated 1/10/20. This is required to renewed every 60 months prior to the current certificate's expiration.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Staff Person #8 and staff person #10 may not work in a childcare position at the facility.

Description: Each staff person meets quals/Transcript, diploma and letters

Noncompliance Area: At the time of the inspection, the foreign education on file for staff person #4 and staff person #7 was not evaluated and translated as required, and therefore, the staff person did not meet the qualifications for the position in which the person is performing. There was no prior childcare experience for staff person #7, staff person #11 and staff person #12. These staff cannot qualify as assistant group supervisors.

Correction Required: A staff person or a substitute staff person shall meet one of the applicable staff qualifications for the position in which the person is performing. A facility person's record shall include acceptable verification of experience, education or training is a transcript or a diploma or a letter signed by a representative of the experiential, educational or training entity.

Regulation: 3270.102(a)

Description: Clean and good repair

Noncompliance Area: During the unannounced inspection on 9/3/24, on the Preschool playground, the blue big wheel bike was cracked by the handlebars and had broken plastic exposed. The plastic picnic table had broken edges along the seats and table corners. The Little Tykes Climber's blue standing base was cracked and was not able to properly support a child standing on it. On the toddler playground, the white plastic table had a crack in the top of it where fingers could be pinched.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

Regulation: 3270.135(a)(3)

Description: Disposable diapers

Noncompliance Area: During the unannounced inspection on 9/3/24, the trash can used for diapering was broken where the lid would not raise without lifting it. This is not considered hands-free and cannot be used for diapering.

Correction Required: If disposable diapers are provided by a parent or by a facility, a soiled diaper shall be discarded by immediately placing the diaper into a plastic-lined, hands-free covered can.
