Noncompliance Area: At the time of the inspection, in the school age room, the covers for the corner mats were torn and the inside foam was exposed making these not able to be easily cleaned.

Correction Required: Furniture must be durable, safe, easily cleaned and appropriate for the child's size, age and special needs.

Noncompliance Area: At the time of the inspection, a highchair in the infant room did not have the T-shaped safety strap.

Noncompliance Area: At the time of the inspection, for child #1's assessment dated 10/15/25, there was no verification that the parents were provided this assessment since no signature was obtained on the parent conference review sheet.

Correction Required: The services to be provided to the family and the child, including the Department's approved form to provide information to the family about the child's growth and development in the context of the services being provided. The operator shall complete and update the form and provide a copy to the family in accordance with the updates regarding emergency contact information in § 3270.124(f) (relating to emergency contact information).

Noncompliance Area: At the time of the inspection, the emergency contact form for child #5 did not include the phone number of the child's physician or source of medical care. The emergency contact form for child #10 did not include the address of the individual designated by the parent to whom the child may be released.

Correction Required: Emergency contact information must include the name, address and telephone number of the child's physician or source of medical care. Emergency contact information must include the name, address and telephone number of the individual designated by the parent to whom the child may be released.

Noncompliance Area: At the time of the inspection, in the Pre-K room, the emergency contact forms for the school age children in care were not present.

Correction Required: When children are in the facility, emergency contact information shall be present in a child care space for children receiving care in the space.

Description: Health information

Noncompliance Area: At the time of the inspection, there was no initial health report on file for child #8 and #9 and they have been enrolled for more than 60 days (see LIS code sheet for date of admission).

Correction Required: The operator shall require the parent of an enrolled child, including a child, a foster child and a relative of an operator or a facility person, to provide an initial health report no later than 60 days following the first day of attendance at the facility.

Description: Mantoux TB/Exam communicable disease

Noncompliance Area: At the time of the inspection, staff person #2 did not have documentation of when a TB test was performed at initial employment (See LIS code sheet for hire date). The health assessment for staff person #2 answered "yes" to communicable diseases with no additional follow up, as required.

Correction Required: An adult health assessment must include tuberculosis screening by the Mantoux method at initial employment. Subsequent tuberculosis screening is not required unless directed by a physician, physician's assistant, CRNP, the Department of Health or a local health department. An adult health assessment must include an examination for communicable diseases and the results of that examination.

Description: Potentially hazardous food refrigerated

Noncompliance Area: At the time of the inspection, in the Toddler Town classroom, numerous lunchboxes were observed in cubbies. Inside these lunchboxes contained yogurt, lunch meat, Lunchables and chicken nuggets. These are required to be refrigerated.

Correction Required: Potentially hazardous food brought from the child's home or provided by the facility shall be refrigerated.

Noncompliance Area: At the time of the inspection, the emergency contact form for child #5 also contained information for the sibling of child #5 and therefore was not an individual record.

Correction Required: An operator shall establish and maintain an individual record for each child enrolled in the facility.

Noncompliance Area: At the time of the inspection, in the Toddler Town classroom, the water for the bathroom sink measured 111° F.

Correction Required: Hot water temperature, in areas accessible to children, may not exceed 110° F.

Noncompliance Area: At the time of the inspection, the baseboard heat vent in the Emerging Preschool classroom measured 213° F and was accessible to children.

Correction Required: Hot water pipes and other sources of heat exceeding 110° F that are accessible to children shall be equipped with protective guards or shall be insulated to prevent direct contact.

Noncompliance Area: At the time of the inspection, in the school age room, water damaged ceiling tiles were present.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.

Description: Evacuation routes posted

Noncompliance Area: At the time of the inspection, evacuation routes were not posted on each floor of the facility.

Correction Required: The Director or designated staff person who is responsible for compliance with this chapter ensure evacuation routes are posted in a conspicuous location on each floor of the facility.
