Findings: The third staff member at this site was moved to a different site at the beginning of the school year and there has been a substitute in place. Today, the substitute was a child care teacher from the Plum Point Elementary BASP site. The substitute at this site was not recorded on the substitute record. When substitutes are used, even if they are qualified child care teachers, and they are not on the staffing pattern at the site or a regularly scheduled staff member, they must be recorded on the substitute record. This was corrected while the Licensing Specialist was on site.

Findings: There is one box of Zyrtec present in the medication box for a child with an allergy. There is a current medication form signed by the health care provider and parent for the Epi Pen that is also present in the box, but the medication authorization form for the Zyrtec was for last school year and is not current. If the child needs to keep the Zyrtec at the child care program, please have the parent obtain an updated medication administration form for this medication, which is signed by the health care provider and the parent. If this medication is no longer needed at the child care program, please return it to the parent. Please send a written plan of correction to the Licensing Specialist. There are two boxes of Albuterol for two other children that have current medication administration forms. There is no expiration date on the Albuterol, but the refill date is 7-2023. Please check with the parent to insure these medications have not expired.

Findings: Licensing Specialist observed several attendance sheets being used at the facility. Licensing Specialist observed one to have parents name, number of children (not name), time in and out and a handwritten date. This seems to be the record the program is using, as this was the record that was on the table and the one that a parent had written on this morning. Licensing Specialist did observe the sign in and out sheet that is provided by the CCPS Child Care office and the substitute Director did sign the children in on this sheet after discussion. Center should be using the sign in and out sheet provided by CCPS so children's attendance is being taken consistently at every program. Using this record should also insure children's whereabouts are known at all times, as this record should be used every time a child enters and leaves the program, whether to go to a club or to their school classroom. Send written correction to Licensing Specialist.
