Findings: During a review of all children's emergency forms, it was found that none of the 10 children enrolled have had their emergency forms updated within a year. In addition, one emergency form does not include a parent's signature or date of the completion of the form, one does not contain the child's address and another does not include the parent's signature. A copy of this list with the child's name and missing information is being provided to the Center so that they may correct this with the parent. Please have the parents add the missing information to their respective child's emergency form and initial and date it for the update. Please then send a copy of the updated forms to OCC for the correction of this non-compliance.

Findings: During the inspection, a hole was found behind the door of the Track Stars (2 year old) room, seemingly caused by the door handle. There are also holes in the wall of the staff bathroom where it seems something used to be hanging. There is also chipping, peeling and deteriorating paint behind the door in the Track Stars room and on the baseboards in the Atrium area. Please repair these areas as to maintain the building in good repair. Please send pictures to OCC to show that the building has been adequately repaired for the correction of this non-compliance.

Findings: Center needs to repair or replace their water cooler so that children age 2 years old and older have water that is safely accessible without assistance from an adult and not located in the bathroom or from a sink used for handwashing. Please send a picture of the new or repaired and properly working water cooler to OCC for the correction of this non-compliance.

Findings: During the inspection, there were no sheets available for the infant cribs or the preschool cots. In addition, one of the moisture-proof mattress coverings in one of the cribs has torn. Please replace the mattress or the moisture-proof covering for the identified mattresses. Also provide proof of having sheets for all of the cots and snugly fitted sheets for all of the cribs. Please send pictures showing evidence of this for the correction of this non-compliance.
