I have used this provider for more than 6 months

We absolutely LOVE this provider. We have no complaints. My kids even ask to go on the weekends! I so happy after having twins she had a spot for all 4 of my children. No wonder she stays full.

2024-09-05 Mandatory Review 13A.15.05.04A(2) Corrected
Findings: The provider had a fire inspection in February 2024. The report indicated that the provider needed to install a "remote smoke alarm" in the living room area. The provider has not done this yet. The provider is advised that she needs to install the smoke alarm in the living room, as that is where the children nap.
2024-09-05 Mandatory Review 13A.15.05.04B(2) Corrected
Findings: The provider has a hard mounted gate at the top of her stairs. The latch part is coming out of the drywall. The provider is advised to fix the latch by either using longer anchoring screws or mount the latch to a piece of wood that is mounted to the drywall.
2024-09-05 Mandatory Review 13A.15.05.05B Corrected
Findings: The provider has a fenced play area in her front/side yard. There are two gates that separate the fenced play area from the back yard, which is off limits. The gates are not aligned with the fence and do not close properly. The provider is advised that she will need to ensure one of the gates closes and latches properly so the children do not have access to the back yard. The long part of the fence closer to the woods has a large gap between the ground and the bottom of the fence that is large enough for a child to crawl though. The provider is advised to close the gap along that side of the fence. The provider noted that the ground in their yard has been shifting, causing these issues. We discussed using mesh fencing, chicken wire, or filling in with dirt. The trampoline is off limits, but the ladder was up. The LS removed the ladder to make the trampoline inaccessible.
2024-09-05 Mandatory Review 13A.15.05.06A Corrected

There is a two year old in care who still sleeps in a pack n play and his did not have a sheet on the mattress pad. The provider has 6 nap mats and uses one for diapering. The provider is advised that she will need 8 total nap mats and a separate diapering mat, otherwise she will be reduced to a total of 5 preschool aged children on her registration.
2024-09-05 Mandatory Review 13A.15.10.01B(1) Corrected
Findings: The provider can not locate her escape plan. Provider is advised to find it or create a new one and ensure it is posted in the child care area.
2023-09-07 Full 13A.15.03.02A Corrected
Findings: There are two children who are missing updated immunizations. There are twin siblings who are missing evidence of any immunizations. Please have the parents obtain updated immunizations for these children. There is one child who is missing a completed Health Inventory Part I completed by the parent.
