Description: Updated every 6 months/Emergency master file updated

Noncompliance Area: On 6/4/26, emergency contact information observed in the childcare spaces where children were receiving care were not updated at least in a 6-month period.

Correction Required: The parent shall update in writing emergency contact information once in a 6-month period or as soon as there is a change in the information. If emergency information is updated in a master file, it shall be updated accordingly in other facility records.

Description: Pertinent Laws & Regulations/General Health and Safety

Noncompliance Area: On 6/4/26, Staff person #1 was observed caring for children unsupervised. Staff person #1 has not completed the following pre-service training required prior to caring for children unsupervised: Mandated Reporter Training and Health & Safety Training Part 1.

Correction Required: A facility shall be operated in conformity with applicable Federal and State laws and regulations.State agencies whose regulations may relate to the operation of a facility include the Department of Environmental Resources, the Department of Labor and Industry, the Department of Health, the Department of Education and the Department of Transportation. Conditions at the facility may not pose a threat to the health or safety of the children.

Description: Health and Safety Training/Comply with CPSL

Noncompliance Area: On 6/4/26, Staff #1 has not completed the required pre-service Mandated Reporter training and Health/Safety Part 1 and Part 2 trainings within 90 days of their date of hire. Staff #2 and Staff #3 have not completed the required pre-service Health & Safety Part 1 and Part 2 Trainings within 90 days of hire. Staff #4 has not completed the required pre-service Health & Safety Part 2 Training within 90 days of hire.

Correction Required: Staff persons shall complete professional development within 90 days of hire as listed in subsections (f)1-10. The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services).

Regulation: 3270.32(a)/3270.192(4)

Description: Comply with CPSL/CPSL information

Noncompliance Area: On 6/4/26, Staff #3 and Staff #4 files did not contain results of NSOR clearance as required by CPSL. Additionally, Staff #1 was observed working unsupervised in Room 1. Staff #1 file did not contain results of out of state clearances required by CPSL. Staff #1 file did not contain requests for the required out of state clearances.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). LACKING REQUIRED HIRING DOCUMENTS: Facility Person # 1 #2 and #3- may not work in a childcare position at the facility. PROVISIONAL HIRE UNSUPERVISED: A provisional employee may not be permitted to work alone with children and must work within the vicinity of a permanent employee. A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Facility Person #1- may not work in a child care position at the facility.

Regulation: 3270.82(d)

Description: Training chair - sanitized

Noncompliance Area: On 6/4/26, upon entering the bathroom located in Room 4, cert rep observed two training chairs containing urine that were not emptied after use. Approximately 1 minute later, Staff #1 was observed emptying the urine in one of the training chairs, but Staff #1 did not sanitize the training chair prior to another child utilizing it.

Correction Required: A training chair is not a flushing toilet. A training chair shall be emptied and sanitized after each use. An acceptable sanitizing solution is 1/4 cup of bleach combined with 1 gallon of water. A sanitizing solution shall be treated as a toxic.

Regulation: 3270.152

Description: Adult Hygiene

Noncompliance Area: On 10/16/25, at time of renewal, Staff #3 was observed changing diapers in the older toddler bathroom. Upon completing the diaper change, Staff #3 did not wash their own hands.

Correction Required: A facility person shall wash his hands before meals and snacks, and after toileting and after diapering a child.

Description: Mixed Age Level/Supervision on and facility premises

Noncompliance Area: On 10/16/25, at approximately 10:30am, Staff #1 and Staff #2 were observed out of ratio with 9 children. The youngest children were infants.

Correction Required: When children are grouped in mixed age levels, the age of the youngest child in the group determines the staff:child ratio and maximum group size in accordance with the requirements in § 3270.51 (relating to similar age level). The required staff:child ratios must be maintained at all times. The requirement for supervision on and off the facility premises includes compliance with the staff:child ratio requirements in §§ 3270.51--3270.55

Description: Locked or inaccessible/Locked

Noncompliance Area: On 10/16/25, at time of renewal, diaper cream and infant teething drops instructing to "keep out of reach of children" were observed accessible to children in the young toddler/infant room. Two tubes of toothpaste instructing to "keep out of reach of children" were observed in a shelf accessible to children in the preschool room.

Correction Required: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children. Medication shall be stored in a locked area of the facility or in an area that is out of the reach of children.

Description: Lidded waste receptacles

Noncompliance Area: On 10/16/25, at time of renewal, the diapering area located in the infant/toddler room was not equipped with a lidded waste receptacle.

Correction Required: A toilet area, training chair area, diapering area and sink area shall be equipped with a clean, lidded waste receptacle.

Description: Devices must be compliant

Noncompliance Area: On 10/16/25, at time of renewal, the fire alarm located in the young toddler/infant building was tested for operability. After the test, the fire alarm was not operable due to a malfunction with resetting the system.

Correction Required: Fire detection devices or systems must be in compliance with standards established under section 1016(c) of the act (62 P.S. § 1016(c)).

Description: Clean, good repair, proper size

Noncompliance Area: On 10/3/24, at time of renewal, a shelf in the young toddler room was not mounted to the wall, causing a possible safety hazard if pulled forward on of a child.

Correction Required: Furniture must be durable, safe, easily cleaned and appropriate for the child's size, age and special needs.

Description: Health information/Initial and subsequent health reports

Noncompliance Area: On 10/3/24, at time of renewal, Child #1 file did not contain an initial health report, including immunization record.

Correction Required: The operator shall require the parent of an enrolled child, including a child, a foster child and a relative of an operator or a facility person, to provide an initial health report no later than 60 days following the first day of attendance at the facility. A child's record shall contain initial and subsequent health reports.
