We added another crib to a classroom and signed it with child's name and labeled one side as "head" and other side as "feet". Moving forward, children will not be present in the classroom and taking naps unless cribs and cots are not signed with children's names and labeled with the position of head and foot.

All agreements now have a written list of services that are provided to children at our daycare. Moving forward, all parents will be filling out the agreement form in its fullness before accepting a child to our childcare facility. If a child receives a child service report it will be added to the file. We did add an IEP document to the file of Zhylych Kevin, who receives child services.

Our plan of correction is to verify the first and last name of a designated person on an emergency contact form and then compare with the contract. We will make sure that the parents include the designed for a pick-up persons on an agreement. As of now, we corrected 1-3 children's agreements and persons designated by a parent for pick-up have been added.

If a parent reports that the child was just changed before arrival, the teacher will still check the diaper at drop-off and, at minimum, check and change as needed every two hours thereafter for every child in the classroom. Teachers write down every diaper check and change on a log for each child, including the date, time, and whether the diaper was dry, wet, or soiled. If a teacher notices a rash or any skin irritation, that gets noted too, so it can be shared with the parent and followed up on. The log will be filled out right when the check or change happens, not later in the day, so the record stays accurate. Parents can look at their child's log anytime, and completed logs are kept on file.

We labeled the nurser with child's #1's name. Effective immediately, all bottles will be printed or hand-labeled with each child's name on a daily basis. Parents pick up their child's bottles at the end of each day to take home for washing. If a child arrives at the center without a labeled bottle, office staff will create and apply a name label before the bottle is placed into use in the classroom.
