Noncompliance Area: At the time of the renewal inspection the certification representative observed that the facility's written policy identifying: the prevention of shaken baby syndrome, abusive head trauma, and child maltreatment; did not include the identification and prevention of child maltreatment.

Correction Required: A facility shall be operated in conformity with applicable Federal and State laws and regulations.State agencies whose regulations may relate to the operation of a facility include the Department of Environmental Resources, the Department of Labor and Industry, the Department of Health, the Department of Education and the Department of Transportation. Conditions at the facility may not pose a threat to the health or safety of the children.

Noncompliance Area: At the time of the renewal inspection the certification representative observed that staff #1 did not have a health assessment on file dated within the last 24 months.

Correction Required: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem.

Description: Pick-up and drop-off points

Noncompliance Area: At the time of the renewal inspection the certification representative observed that the provider did not notify local traffic safety authorities annually in writing of the location of the facility and the program's use of pedestrian and vehicular routes around the child care facility.

Description: Training regarding plan

Noncompliance Area: At the time of the renewal inspection the certification representative observed that staff #1 did not have documentation of training on the emergency plan.

Correction Required: The operator shall assure that each facility person receives training regarding the emergency plan at the time of initial employment, on an annual basis and at the time of each plan update. The operator shall document the date of each training and the names of all facility persons who received the training and kept on file at the facility.

Description: Emergency plan

Noncompliance Area: At the time of the renewal inspection the certification representative observed that the provider did not have verification of submitting a copy of the facility's emergency plan to the local municipality.

Correction Required: The operator shall send a copy of the emergency plan and subsequent plan updates to the local municipality and to the county emergency management agency.

Description: Every 60 days

Noncompliance Area: At the time of the renewal inspection the certification representative observed that the facility did not have documentation of a fire drill within the last 60 days. The last documented drill was September 2025.

Correction Required: The Director or designated staff person who is responsible for compliance with this chapter ensure fire drills are conducted at least once every 60 days.

Description: Consent for emergency medical care required prior to admission

Noncompliance Area: At the time of the renewal inspection the certification representative observed that child #1 did not have signed parental consent for emergency medical care.

Correction Required: A child's record shall contain signed parental consent for emergency medical care for the child. Written consent is required prior to admission.

Noncompliance Area: At the time of the renewal inspection the certification representative observed that child #1 did not have signed parental consent for administration of minor first- aid procedures by facility staff.

Description: Training regarding plan

Noncompliance Area: At the time of the renewal inspection the certification representative observed that staff #2 did not have documentation of the completion of emergency plan training in the last 12 months.

Correction Required: The operator shall assure that each facility person receives training regarding the emergency plan at the time of initial employment, on an annual basis and at the time of each plan update. The operator shall document the date of each training and the names of all facility persons who received the training and kept on file at the facility.

Description: Fire safety - 1 yr.

Noncompliance Area: At the time of the renewal inspection the certification representative observed that staff #2 and #5 did not have the verification of the completion of fire safety training.

Correction Required: Staff persons shall participate, at least annually, in firesafety training conducted by a fire protection professional. Staff persons and volunteers shall receive training in maintenance of smoke detectors, the duties of facility persons during a fire drill and during a fire and the use of the facility's fire extinguishers, not including discharge of the fire suppressant agent.

Description: Comply with CPSL

Noncompliance Area: At the time of the renewal inspection the certification representative observed that staff #1 had an FBI clearance that was more than 60 months old (dated 7/23/25); staff #4 did not have out of state clearances from New Jersey (has current NJ ID) and Delaware (has a DE clearance request from 2023 and went to college in DE).

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). SAFETY PLAN: The operator is required to submit to the Children and Youth Agency a plan of supervision or alternate arrangement as required by the Child Protective Services Law. LACKING REQUIRED HIRING DOCUMENTS: Facility Person # -- may not work in a child care position at the facility. PROVISIONAL HIRE UNSUPERVISED: A provisional employee may not be permitted to work alone with children and must work within the vicinity of a permanent employee.

Noncompliance Area: At the time of the renewal inspection the certification representative observed that the facility did not have the Department's approved form to provide information to the family about the child's growth and development in the context of the services being provided.

Noncompliance Area: At the time of the renewal inspection the certification representative observed that the facility did not complete the civil rights compliance questionnaire.

Correction Required: A legal entity must comply with The Pennsylvania Human Relations Act (43 P. S. §§951-962.2), the Age Discrimination Act of 1975 (42 U.S.C.A. §§6101-6107), Title VI of the Civil Rights Act of 1964 (42 U.S.C.A. §§2000d-2000d-4a).
