| Findings: The director faxed a reportable child incident report. The report states that a staff spilled hot water on the baby's foot, causing burns. Parents were called and 911. The ambulance came to get the baby. The staff member involved in the incident was interviewed via telephone on October 30, 2024, and submitted a statement to the specialist on October 31, 2024. Please see the remarks. The OCC learned from another state agency that a staff member at the childcare center wiped a child's nose with a magic marker eraser to remove the marker from the child's nose, resulting in a chemical burn. The OCC does have an incident report on file from the childcare center regarding this incident on July 27, 2023. The center terminated the staff member involved in this incident. At the time of the inspection, the director and staff members 2 and 3 were interviewed. Staff members 2 and 3 stated that the director informed them that water was spilled on an infant's foot. Once the director observed the baby's foot 911 and the baby's mom were called according to staff members 2, 3, and the director. | Findings: The complaint alleges that on June 24th a child's parents saw their child riding a pony at the center. Reportedly, there was no notice or permission slips given to the parents that animals were going to be present at the center which upset parents whose children have allergies. The director states that a petting zoo came to the center for about 45 minutes and this activity is usually for the summer camp children. She states that the 2-year-old class was outside for their outdoor play, and the teachers in this classroom brought the children over to the area where the petting zoo was and allowed the children to see and touch the animals. The director states that some of the children in the 2-year-old class did ride the pony and photos of this were posted on the BrightWheel App. The child's parents liked the photo of the child riding the pony, and there were no complaints received from any of the parents about their children riding the pony, or interacting with the petting zoo according to the director. The director stated that parents receive a calendar and newsletter that contains activities going on at the center, and it is posted on the parent board. See remarks. | |||

| Findings: The complaint alleges that in late January or early February, a sign was posted on a classroom door at the center, which read 'PARENTS PLEASE DO NOT ENTER THE CLASSROOM drop off and pick up at the door thank you'. Reportedly, on the first day that the sign went up, the director physically blocked the entryway to the classroom. The director was interviewed and stated that this sign was posted on the door due to the child's parents hanging out in the classroom, distracting and disrupting the teachers and students during drop-off and pick-up. She states that the parents would hang out in the classroom for about 8 minutes. The director states that parents were still allowed to drop off and pick up inside of the classroom after the note was posted on the door, and they did. She also states that she had a conversation with the parents regarding the distractions caused by hanging out in the classroom and the parents stated that they understood. | Findings: There is a staff person working today who just started today. She has cleared background check results, but the release was just given to Licensing during the inspection today. The release was sent electronically to the Office to be cleared during the inspection. There are two other staff who have been working at the center, one that the OCC has not been notified about. The center has not given releases to the OCC for either staff. Please ensure that the OCC is notified within 5 days of the addition of a new employee with a 1203 form, that the OCC is given a notarized release so that it can be cleared before the staff person begins working, and that the staff person also completes a criminal background check before they begin working. | |||
