Initial Comments: The following deficiencies were found at the time of the Compliance inspection conducted on 9/19/25 and are subject to changes pending programmatic review. The Licensee will be notified when the Written Documentation is requested to be submitted through the Licensing Portal. A copy of the Notice of Inspection Rights was provided at the time of the Inspection. The following was discussed, but not limited to: 1) New rules effective 8/3/25, 2) Tummy time rules, 3) Expulsion/ suspension policy is now required in the statement of services, 4) Anniversary fee due by 11/30/25, and 5) Contacting the State Fire Marshall to verify which exits in the licensed classrooms are the designated exits and ensuring any designated exit is unlocked and unobstructed during hours of operation. There were 4 staff files reviewed. 2 of the 4 fingerprint clearance cards were verified to be valid through the DPS website.

Initial Comments: The following deficiencies were found at the time of the Compliance Inspection conducted on 10/4/2024, and are subject to changes pending programmatic review. Name of Compliance Officer: Jennifer Flicker The Written Documentation is required to be submitted through the Licensing Portal within 10 days of the receipt of the Statement of Deficiencies. The following was discussed but not limited to: 1) The license anniversary fee due by November 30, 2024, 2) The change of space utilization application that is required due to the room changes made, 3) Updating room capacities related to the modification, 4) Tummy time rules was discussed with infant room staff, 5) Items stored in a bathroom are required to be stored inaccessible to children and in either a closed cabinet or on a shelf in a covered waterproof container, and 6) Submitting the current fire inspection when obtained. There were 4 staff files reviewed. 4 of the 4 fingerprint clearance cards were verified to be valid through the DPS website. The Empower Assessment was completed at the time of the inspection.

Initial Comments: The following deficiencies were observed at the time of the Compliance Inspection conducted on 10/25/2023 and are subject to changes pending programmatic review. Please submit the Plan of Corrections via the LMS portal within 10 days. 7 of 7 Fingerprint Clearance cards reviewed were valid via a DPS website search. The Empower self-evaluation was completed at the time of the inspection. The Emergency Disaster Plan update form was completed at the time of the inspection. The following was discussed but not limited to: 1. Sanitizing spray for diaper tables to be determined. 2. Disenrollment policies in the Statement of services. 3. Infant feeding instructions (updated, specific items). 4. Plunger in the toilet room. 5. Documentation of current and continuous enrollment in high school for assistant teachers caregivers.
