The medication is current and labeled as required, but the medication administration form in the child's file is from 2022. These forms are only good for one year. Please obtain an updated medication administration form or allergy-anaphylaxis action plan form, that is completed by the child's health care provider, from the child's parents-guardians and add to child's file. Please send a copy to the Licensing Specialist.
