| Findings: The OCC received a complaint concerning staff child ratios in the mornings. It was reported that there are not enough staff for the amount of children present at the center until about 9am once all the staff arrive. All staff were interviewed. The staff report that the 3s classroom is the opening room for children ages 2 to School Age. It was reported that there are about 15 or 16 children in the classroom with 2 teachers. Then there is only one teacher in the room from 815-830 until the next staff person arrives. The staff reported that there were sometimes 5 two year old children in the opening classroom until around 9am, when their teacher takes them to their 2s classroom. The director and admin were interviewed and reported that the morning routine was recently changed to prevent ratio issues and that staff know to notify the director or admin if there are ratio issues, as they have an extra floater in the morning as well as the director or admin staff. There were 13 children with 2 twos and 2 staff in the opening room this morning when the LS arrived. | |||

| Findings: The OCC received a complaint concerning a child who was observed in the toddler room to have the cord from a speaker wrapped around their neck. The Licensing Specialist observed all classrooms and interviewed all staff. The staff report that the two year old class walks through the toddler room to access the infant toddler playground. The staff with the twos class walked in the Toddler room and noticed that a toddler had a speaker cord wrapped around his shoulder area and was trying to get untangled. The speaker was plugged into the wall and sitting on the window sill. The staff with the twos assisted the child in getting untangled and then moved the speaker to the counter where it was out of reach of the children. The staff with the twos class reported that the Toddler teacher was sitting at the table with the two other toddlers in the classroom and her back was turned away from the child who was by the window. They reported that she did not see or react to the child being tangled. The toddler teacher reported that she did see the child and reacted immediately, untangling the child and moving the speaker. The speaker has been moved and is no longer a potential hazard. | |||
