Noncompliance Area: At the time of renewal inspection observed that emergency contact forms does not accompany staff when children leave the facility for walks. Additionally, the Brightwheel app used by the provider does not contain all of the information required on the emergency contact forms.

Correction Required: When children leave the facility on walking and riding excursions, emergency contact information specific to each child on the excursion shall accompany a staff person on the excursion.

Noncompliance Area: At the time of unannounced inspection on 9/29/25observed visible signs of water damage on the ceiling tiles in the main room.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.

Noncompliance Area: At the time of unannounced inspection on 9/29/25, peeling paint was observed on both sides of the bathroom door.

Correction Required: Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility.

Description: No ridicule or threaten harm/No harsh language

Noncompliance Area: At the time of inspection, Child #1 overheard inappropriate/foul language directed at their parent. Per Manager, the language was used by the individual Staff Person #1 was speaking with on speakerphone while discussing the child's family.

Correction Required: A facility person may not single out the child for ridicule, threaten harm to the child or the child's family and may not specifically aim to degrade the child or the child's family. A facility person may not use harsh, demeaning or abusive language in the presence of children.

Noncompliance Area: At the time of renewal inspection, file of staff person #1 included PSP criminal history that was not complete and had "request under review for control" status. File of staff person #1 did not include current PA child abuse clearance.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Staff Person # 1 must be removed from child care position by close of business. PA child abuse clearance and updated state police clearance must be obtained before facility person #1 may resume a child care position.

Description: Handwashing signs

Noncompliance Area: At the time of renewal inspection, a sign on which requirements to wash hands after toileting, diapering and before eating is written was not posted at the sink in the facility.

Correction Required: A facility person and an able child shall wash his hands after toileting and before eating. A sign on which this requirement is written shall be posted at each toilet, training chair, diapering area and sink in the facility.

An acceptable letter of exemption was not observed in the file of child #2, #3, and #4 .

Correction Required: A health report shall include a review of the child's immunized status according to recommendations of the ACIP. Exemption from immunization for religious belief or strong personal objection equated to a religious belief shall be documented by a written, signed and dated statement from the child's parent or guardian. The statement shall be kept in the child's record.

Noncompliance Area: At the time of renewal inspection, file of staff person #1, #2, #3, #4, and #5 did not include Pennsylvania Health and Safety Update 2022 1 hour professional development training required by OCDEL within the prescribed timeframe which is by December 30, 2022.

Correction Required: A facility shall be operated in conformity with applicable Federal and State laws and regulations.State agencies whose regulations may relate to the operation of a facility include the Department of Environmental Resources, the Department of Labor and Industry, the Department of Health, the Department of Education and the Department of Transportation. Conditions at the facility may not pose a threat to the health or safety of the children.
