Findings: During a review of children's records, it was found that a 17 month old girl and a two year old girl have no evidence of having any blood lead testing. Please notify the parents in writing of the need for blood lead testing and provide a copy of that notification to OCC for the correction of that portion of this non-compliance. That is with the understanding that the Provider will obtain the blood lead test results and place them in the respective children's file. It was also found that two children in a sibling group do not have the proper amount of age-appropriate immunizations on file. Please inform the parent of the need for age-appropriate immunizations and the immunization record. Please forward the updated immunization records to OCC for the correction of this portion of the non-compliance.
