This may be missing due to her having only just turned one year old approximately two weeks ago. In addition, a 1 year and 4 month old girl is missing her Health Inventory Part I and another DTP vaccination. Please inform her parent of this missing documentation and immunization so that the parent can complete, sign and date the form and return it to you as well as obtain another age-appropriate vaccination for her child or the documentation of already having it. A two year old girl is missing documentation of any blood lead testing. Please inform the parent of this so that they may either provide you the results or schedule her test and provide the results. After receiving the documentation, please keep a copy for their file and send a copy to OCC for the correction of this non-compliance.

Findings: During the inspection, a pillow and a thick fluffy blanket were found in two of the three pack-n-plays. Licensing Specialist educated the Provider about the dangers of such objects as they are a suffocation hazard. The Provider removed all of the items from both of the pack-n-plays during the inspection. Therefore, this non-compliance was corrected on-site.

Findings: The provider didn't produce evidence that she completed 12 hours of training between November 2019 and November 2020. Due to the pandemic providers were given a grace period to complete their training but it was to be completed by December 2020. The provider completed a 25 hour training on 1/17/21. 12 of these 25 hours were late but can satisfy the training for the 2019-2020 year and the remaining 13 hours will satisfy the Nov 2020-Nov 2021 training year. The non-compliance for not completing the Nov 19-Nov 20 training timely was corrected by 1/17/21, however the provider didn't forward a copy of the training certificate until today during the inspection.
