This may be missing due to her having only just turned one year old approximately two weeks ago. In addition, a 1 year and 4 month old girl is missing her Health Inventory Part I and another DTP vaccination. A two year old girl is missing documentation of any blood lead testing. Please inform the parent of this so that they may either provide you the results or schedule her test and provide the results. Due to the pandemic providers were given a grace period to complete their training but it was to be completed by December 2020. The provider completed a 25 hour training on 1/17/21. 12 of these 25 hours were late but can satisfy the training for the 2019-2020 year and the remaining 13 hours will satisfy the Nov 2020-Nov 2021 training year.
