The fingerprint clearance cards for 2 of 2 staff members were verified to be valid through the DPS website at the time of the inspection. During the exit interview, the following items were discussed, but not limited to: *Discussed the new ruleset and where to find new documentation. *Discussed that each enrolled child must have all the correct information located on one Emergency, Information & Immunization Record card. *Continue to move the sand around to meet the 6" of resilient surfacing under the fall zones in the playground.

Please submit the Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. ***Make sure to send the updated violation free fire inspection report. The Emergency Disaster Contact Form was completed at the time of the inspection. The fingerprint clearance cards for 2 of 2 staff members were verified to be valid through the DPS website at the time of the inspection. During the exit interview, the following items were discussed but are not limited to: *Ensure there is 6" of resilient surface in the areas of fall zones. *Repair the peeling vinyl on the climber's steps.
