Initial Comments: The following deficiencies were observed at the time of the Compliance Inspection conducted on 11/19/2025 and are subject to changes pending programmatic review. The Compliance Officer provided a paper copy of the Notice of Inspection Rights and the Small Business Bill of Rights to the Facility Director at the time of the inspection. Please submit the Plan of Corrections via the LMS portal within 10 days. Vehicle #27A 72Z was inspected and is approved to be used to transport enrolled children. 3 of 3 Fingerprint Clearance cards reviewed were valid via the DPS website search. The Empower self-evaluation was completed at the time of the inspection. The Emergency Disaster Plan update form was completed at the time of the inspection. The DES Group Size requirements were observed to be compliant at the time of the inspection. The following was discussed but not limited to: 1. Spacing of cribs in the Infant room. 2. Tummy time location in the room. 3. Brooms, dust pans, and the perforated signs (with sharp edges) must be inaccessible to children. 4. Soiled diaper containers must have a tight-fitting lid. 5. Van maintenance and cleanliness.

Initial Comments: Statement of Deficiencies Amended on 12.20.24 The following deficiencies were observed at the time of the Compliance Inspection conducted on 11/26/2024 and are subject to changes pending programmatic review. Please submit the Plan of Corrections via the LMS portal within 10 days. ***The Director was reminded of the license expiration date and to log on to the portal to pay the fees prior to the license expiring. 2 of 2 Fingerprint Clearance cards reviewed were valid via the DPS website search. The Empower self-evaluation was emailed to the Owner at the time of the inspection. The Emergency Disaster Plan update form was completed at the time of the inspection. The DES Group size checklist was completed at the time of the inspection. Vehicle HXA1GF was inspected by CO #2 and is approved to be used to transport enrolled children. The following was discussed but not limited to: 1. Ensure diapering procedures are posted in each diapering area. 2. Ensure toilet room vents are maintained in a clean condition. 3. Ensure water and cups are accessible to enrolled children. 4. Ensure staff are documenting times of departure each day. 5. Ensure infant feeding plans are current. 6. Ensure training hours are verified for substitute staff. 7. Ensure the diaper surface is located adjacent to the water source. 8. Ensure the capacity is posted in each activity area. 9. Initial Comments: The following deficiencies were found at the time of the Compliance Inspection conducted on 12/8/2023, and are subject to changes pending programmatic review. Name of Compliance Officer: Jennifer Flicker The Written Documentation is required to be submitted through the Licensing Portal within 10 days of the receipt of the Statement of Deficiencies. The following was discussed at the time of the inspection: 1) The recently purchased climbing mat in the infant room can not be used for tummy time due to the mat's lack of firmness, and 2) Removal of the plastic straps near the paper roll slot on the 3 newly installed diapering counters. There were 4 staff files reviewed. 4 of the 4 fingerprint clearance cards were verified to be valid through the DPS website. The Empower Survey was completed by the Director during the inspection.

Initial Comments: Based on Surveyor's observation, a 40 inch TV had been placed on top of a 2 foot high shelf. The TV was not mounted and wobbled when the Surveyor pushed on it.

Initial Comments: Based on Surveyor's observation and a discussion with the infant staff and the Director, the staff and Director acknowledged that one of the infant's written feeding instructions for a child who was present was missing and was not posted in the kitchen or in the infant room, as required.

Initial Comments: Based on Surveyor's observation and on a discussion with the infant room staff, a bottle was propped on an infant's chest as the infant was laying down in a crib. The teacher stated that she was working on helping the infant hold onto the bottle but stated she did not know a bottle could not be in the crib and that bottles can not be propped.
