| Findings: During a review of all children's emergency forms, it was found that a two year old boy's date of birth was incorrectly written; indicating that he is only 4 months old. That same child's emergency form is missing his physician's full address. Two other children's emergency forms are also missing their physician's addresses. Those two children are both two year old girls. Please have the parents complete the missing information on their respective child's emergency form, initial and date the form for the update. Send a copy of the emergency forms including the added information to OCC for the correction of this non-compliance. | |||
