Corrective Action: Ensure attendance accurately reflects the physical classroom staff and children are present in. LS Kalafut reviewed the attendance records from the date of the visit. Eleven children were present in the one and two-year-old classroom. Five children were signed onto the one and two-year-old attendance record and five were signed onto the three and four-year-old attendance record. LS Kalafut reminded Kana attendance should reflect the exact room each child is in. Kana corrected the attendance records by signing the five three and four-year-olds out on the three and four-year-old classroom attendance record and signing them onto the one and two-year-old attendance record. Kana then signed the remaining child onto the record reflecting the eleven children present in the one and two-year-old classroom.

Corrective Action: Remove the pressure gate used between the one and two year old nap room and back hallway from the center. Supervisor, K.Garvey, observed the gate fall on top of a child. Replace this gate with an installed and securely anchored gate to prevent falling.

Without employment history, service letters do not need to be send. Replace service letters with two additional references; four total.
