Findings: LS conducted complaint investigation on 09/02/2025. Caller stated that her 3 year old grandson lost 2 front teeth. It was reported that the child took a block away from another child, who then pushed him into a sensory alligator toy. Provider must aways report a injury to a child while in care that results in treatment by medical professional within 24 hours to the OCC.

Findings: LS discussed with provider to always supervise all children in care while at the family daycare at all times. In April of 2025 the bottom gate, was unlatched and a child wondered upstairs to the main level and was found in the bed with the provider's father. Provider was asked by the CPS worker to explain the events from that incident. The provider stated that she was made aware from her substitute that the child was missing. The provider stated that she then noticed that the gate was left unlatched after a parent left the daycare. A resident of the house text the provider a picture of the child in the bed with her father. The provider then went up the stairs to get the child. The provider stated that after the incident she made the family aware. Per the provider the child was upstairs for three minutes.
