Noncompliance Area: SECOND CITATION. At approximately 2:22pm Staff Person #1 was observed to be supervising 4 children identified as 1 preschool age and 3 older toddlers in the Two's Classroom. A child was observed to be placed between a shelving unit and a table. Staff Person #1 was unable to see the child from their position at a table in the classroom. There was also a lack of appropriate lighting to allow Staff Person #1 to properly see and assess the children on the far side of the classroom from their position at the table. This did not allow for appropriate supervision. FACILITY PREVIOUSLY CITED ON 6.15.26.

Correction Required: 1. Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. This part of the plan will have an immediate correction date. 2. The legal entity must arrange for all facility staff to complete the following supervision trainings through Penn State Better Kid Care: "Supervision: Positioning -- Where Do I Stand?" and "Supervision: What's Required?" Operator will provide a correction date as to when these trainings will be completed.

There was a source of natural light present from the door, but it was not sufficient to properly see and assess the children on the far side of the room from the Staff Person's position at the table. FACILITY PREVIOUSLY CITED ON 6.15.26.

Correction Required: 1. Rooms, hallways, stairways, outside steps, porches and ramps shall be lighted by artificial or natural light. This part of the plan will have an immediate correction date. 2. Operator will identify an appropriate and sufficient source(es) of lighting that will be present during naptime in all classrooms. These appropriate light sources will allow staff to see and assess the children they are responsible for. A sign will be posted in a conspicuous space in all classrooms where napping occurs that identifies the appropriate light source that will be in use during naptime. Operator will provide a correction date as to when this will be completed. 3. Operator will review the identified appropriate light source to be in use during naptime with all staff. At approximately 1:28pm Staff Person #1 and #2 were observed on the eating side in the One's Classroom. One preschooler, 3 older toddlers and 1 young toddler were observed to be sleeping on the opposite end of the classroom. There was insufficient natural or artificial lighting to allow for Staff Person #1, who identified being responsible for the children, to supervise appropriately as they could not see them.

Correction Required: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. Children must be supervised at all times.
