| Findings: OCC received information from the Asst Director reporting a child was left, unsupervised, on a van when coming to the center after school. The van driver 'unloaded the van and walked the kids inside'. The van driver didn't realize the child didn't get out of the van. After approximately five minutes, the child got out of the van and knocked on the back door, but staff did not hear him. An employee from a nearby business saw the child, contacted the center and stayed with him until ctr staff assumed supervision of the child. Asst. Director contacted the child's parents to make them aware of the incident. Asst Director reported 'roll call' policies and practices have been reviewed with staff and the child has been moved to the front seat of the van. LS spoke with the van driver today. She reported the same information that was reported by the Asst Director. The driver stated there were 9 children on the van that day. The child was not feeling well and she was not feeling well. Children got out of the van and driver walked inside with them. Driver stated it was less than 5 minutes when the other store employee called and she went out to get the child. Driver stated child was fine. | |||

| Findings: Van driver-Aide and Asst Director's 1st Aid and CPR expired in October. The van driver transported a group of children today and Asst Director does transport, as well. When LS arrived at the facility, the Asst Director and a substitute were present. The substitute has current 1st Aid and CPR, so there was appropriate coverage at the facility. Currently, the Director, one of the school age teachers and the substitute have current 1st Aid and CPR. The van driver, who is also an aide, and the Asst Director, who is also a school age teacher, need to obtain current 1st Aid and CPR ASAP, since they both transport children.
