Findings: The provider is currently using a new additional adult because the former one recently quit, however the new additional adult has not been approved for care. The provider did previously contact this specialist in regards to getting another additional adult cleared for care but should have been using an approved substitute until then. When this specialist asked the provider about her substitutes, she stated that she will be using all new substitutes. Once OCC clears the additional adult for care, a 3 to 4 Infant orientation has to be conducted at the provider's home. Correct Immediately. OCC will try to clear the new intended additional adult for substitute use ASAP in the meantime. Provider informed the additional adult and all substitutes can not be used for care or left unattended with any children in care until they are approved by OCC.

Findings: The provider did not have her 12 hours of training from last year for this specialist to see. She stated she was unsure of where she put them and how she thought she sent them to the former specialist.

Findings: This specialist observed the fence on the one side of the yard to be broken and bent in some areas. The provider states a tree fell in her yard on this past Halloween and her neighbor is still waiting to have the area assessed. There was no gate in place at the top of stairs that lead to the basement door. There was a storage unit in the yard that contained hazardous sprays/items and there was no lock in place to restrict children from being able to get in it. This specialist also observed a gap between the step and the porch where a child could potentially fall/climb through resulting in a very steep fall. The wood cover to the sand box is broken and bent. The AC until is not covered. The outdoor activity are shall be free from conditions that may be dangerous to the health and safety of the children in care. Correct Immediately. The fence needs to be replaced, the gate at the top of the stairs needs to be fixed, the storage unit shall be restricted or the hazardous items shall be removed, the gap between the one stair and porch shall be filled or fenced in (the back) so a child can not fall/climb through, and the AC unit shall be screened at the top or inaccessible.
