Correction Required: A facility person providing direct care who comes into c WHICH IS contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem.A facility person's record shall include a written report of initial and subsequent health assessments, including the results of initial and subsequent tuberculin skin tests, x-rays or other medical documentation necessary to confirm freedom from communicable tuberculosis.

Correction Required: A facility person's record shall include two written, nonfamily references from individuals attesting to the person's suitability to serve as a facility person

Noncompliance Area: ON THE DAY OF VISIT OBSERVED THE FILE FOR STAFF#2 WHICH DID NOT HAVE AN INTIAL EMERGENCY PLAN TRAINING IN THE FILE.

Correction Required: The operator shall assure that each facility person receives training regarding the emergency plan at the time of initial employment, on an annual basis and at the time of each plan update. The operator shall document the date of each training and the names of all facility persons who received the training and kept on file at the facility.

Noncompliance Area: ON THE DAY OF VISIT OBSERVED THE FILES FOR STAFF#3 WHICH DID NOT HAVE PROOF THE STATE POLICE CLEARANCE IN THE FILE FOR THE STAFF HIRED ON 9/9/19.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). LACKING REQUIRED HIRING DOCUMENTS: Facility Person # 03 may not work in a child care position at the facility. PROVISIONAL HIRE UNSUPERVISED: A provisional employee may not be permitted to work alone with children and must work within the vicinity of a permanent employee. A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Facility Person #03 may not work in a child care position at the facility.

Noncompliance Area: ON THE DAY OF VISIT OBSERVED OUTDOOR PLAY SPACE THERE IS A PLAY HOUSE WHICH HAVE DRY ROTTEN WOOD ON THE FLOOR OF THE. THERE IS A TREE STUMP WHICH HAVE JAGGED WOODED EDGES WHICH CAN BE HAZARDOUS IN THE PLAY YARD.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.

Noncompliance Area: ON THE DAY OF VISIT OBSERVED THE ELEPHANT CLASSROOM WHICH THE FLOOR WAS DIRTY THROUGHOUT THE CLASSROOM.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.

Noncompliance Area: ON THE DAY OF VISIT OBSERVED THE FILE FOR STAFF#1 HIRED 10/31/18 DID NOT HAVE PROOF OF THE STATE POLICE CLEARANCE AND THE FBI CLEARANCE IN THE FILE.

Noncompliance Area: ON THE DAY OF VISIT VERIFIED THAT CHILD#1 RECEIVED A SCRATCH OVER THE EYE AND STAFF COULD NOT TELL INDIVIDUAL#2 WHAT HAPPEN.

Correction Required: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises.

Noncompliance Area: ON THE DAY OF VISIT VERIFEID INDIVIDUAL#1 DID NOT RECEIVE AN INCIDENT REPORT AT LEAST ON 1 INCIDENTS THAT OCCURED AT THE DAYCARE CENTER.

Correction Required: A child's record shall contain reports of accidents, injuries and illnesses involving a child in care at the facility. The original report shall be given to the parent on the day of the incident. The second copy of the report shall be retained at the facility in an accident file. The third copy of the report shall be retained at the facility in the child's file.

Noncompliance Area: During initial inspection on 06/19/18, Room #1 did not have sufficient play equipment that was appropriate for the developmental needs and interests of children to be serviced in room. Children to be serviced will be 3-5 years old, the room only contained board games.

Correction Required: Play equipment and materials appropriate to the developmental needs, individual interests and ages of the children shall be provided in sufficient amount and variety to preclude long waits for use. Play equipment shall facilitate the child's emotional, cognitive, communicative, perceptual-motor, physical and social development.

Noncompliance Area: During initial inspection on 06/19/18, desks and chairs located in Room #1 were covered in dust and needed to be cleaned; sink cabinet door on right side in Room #3 was not properly secured and posed a potential hazard to children.

Correction Required: Furniture must be durable, safe, easily cleaned and appropriate for the child's size, age and special needs.

Noncompliance Area: During initial inspection on 06/19/18, temperature in refrigerator located in kitchen read 50 degrees and had areas of rust and spillage located on the bottom of the refrigerator.

Correction Required: A facility shall have an operable, clean refrigerator used to store potentially hazardous foods. The refrigerator shall be capable of maintaining food at 45 F or below. An operating thermometer shall be placed in the refrigerator.

Noncompliance Area: During initial inspection on 06/19/18, written plan of daily activities was not posted in Rooms #1 and #2.

Correction Required: A written plan of daily activities and routines, including a time for free play shall be established for each group. The plan shall be flexible to accommodate the needs of individual children and the dynamics of the group. The written plan of daily activities shall be posted in the group space.

Noncompliance Area: During initial inspection on 06/19/18, file of staff person #1 did not contain a current health assessment with TB test and subsequent results.

Correction Required: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem.An adult health assessment must include tuberculosis screening by the Mantoux method at initial employment. Subsequent tuberculosis screening is not required unless directed by a physician, physician's assistant, CRNP, the Department of Health or a local health department.
