| Findings: Serious Injury and General Safety Concerns. There were concerns a child had sustained a serious injury to the mouth and parents were not notified. During an investigation, it was verified a child had knocked out a tooth while playing on the monkey bars. The child reported to the staff a tooth had fallen out. It was cleaned up with no further issue. The staff member filled out an incident report and told their supervisor but did not report to the parents. Additionally, it was determined the child needed a dentist's care for the damage to their mouth. Resolution: The provider has updated their policy to notify parent when an incident occurs. | |||
