| Findings: Compliant investigation conducted on 09/22/2025, complaint alleged child sustained an fractured and sprained on there right elbow. LS was accompanied by the Detective and CPS representative. The director and teacher that were referenced in the complaint were interviewed. Per director, she as not made aware that any incident had occurred until she spoke with the teacher. Per director, she received an email from the parents around 8:50 pm that the child had a broken elbow. Director called back in about 10 minutes and asked how it happened. The director stated that the child's grandmother picked her up and the child was holding her arm. The Director stated she then called the teacher, and she stated the teacher said that the child was acting different. During the time of the incident the teacher stated that she was by herself with at least 16 to 18 children. LS observed room 4 where the three and four year old children were combined on the day of the incident. All children are must be appropriately supervised at all times. | |||

During this game the children make themselves fall on the floor or sometimes the children will make themselves fall out there chairs. Teacher also mentioned that sometimes the children will fall on top of each other. Per teacher, the children did this multiple times on the date of the incident but could not state how many times. Complaint alleged that there was a dank smell of water and the gnats are always flying around and bugging both the teachers and the students. The center also has a poor plumbing system. If you inspect close enough you will find tainted food because of the rodent problems. LS observed the food storage closet with rodent droppings on the shelves and floor where the food is kept. LS observed food storage closet with a plywood covering the existing ceiling that was wet, damp and sagging. LS observed evidence of plywood stained with water.
