Findings: During a review of children's records, it was found that a five year old girl is missing her Health Inventory Part I that should be completed by the parent. Three year old and 1 year 7 month old siblings both need a blood lead test. During the inspection, the Provider texted the parent to inform her about the missing tests. The parent reported that she has an appointment for both of them on 08/14/2024. The appointment will encompass their blood lead tests and she will pick up a copy of their most current immunizations. The immunization records that the Provider currently has shows them as being behind on immunizations, but they are reportedly not behind; only missing the documentation. It was also discovered that a five year old girl is missing another MMR immunization and a one year old girl is missing an MMR and a Varicella immunization according to their ages. Please notify the respective parents of the missing documentation and when received, send a copy of the documents to OCC for the correction of this non-compliance.
