A six year old boy and male siblings, ages 1 year and 7 months old and three years old, are all missing their physician's address on their emergency forms. A one year old girl's emergency form is missing her parent's signature and date. In addition, the same one year old girl does not have any emergency contacts listed on her emergency form. | Findings: During a review of children's records, it was found that a five year old girl is missing her Health Inventory Part I that should be completed by the parent. Three year old and 1 year 7 month old siblings both need a blood lead test. During the inspection, the Provider texted the parent to inform her about the missing tests. The parent reported that she has an appointment for both of them on 08/14/2024. The appointment will encompass their blood lead tests and she will pick up a copy of their most current immunizations. The immunization records that the Provider currently has shows them as being behind on immunizations, but they are reportedly not behind; only missing the documentation.
