Noncompliance Area: During the renewal inspection completed on 8/18/26 the file reviewed for staff person #1 did not include an updated health assessment.

Correction Required: An adult health assessment must include a physical examination. A facility person's record shall include a written report of initial and subsequent health assessments, including the results of initial and subsequent tuberculin skin tests, x-rays or other medical documentation necessary to confirm freedom from communicable tuberculosis.

Noncompliance Area: During the renewal inspection completed on 8/18/26 there was missing receptacle cover observed in stairway leading to 2nd floor accessible to children.

Correction Required: Protective receptacle covers shall be placed in electrical outlets accessible to children 5 years of age or younger.

Correction Required: A first-aid kit must contain the following: soap, an assortment of adhesive bandages, sterile gauze pads, tweezers, tape, scissors and disposable, nonporous gloves.

Description: Similar Age Level

Noncompliance Area: During the unannounced monitoring on 8/26/25 the toddler classroom was observed to be out of ratio. There were 16 children present with staff person #1 and staff #2; youngest child in the group was 2 years old. The activity room was observed to be out of ratio. There were 5 children present with staff person #3; youngest child in the group was an infant. Continued noncompliance from the renewal inspection on 8/12/25.

Correction Required: 1) The required staff: child ratios must be maintained at all times. 2) All staff must receive minimum of two hours of training regarding maintaining ratio. Certification representative will submit referral to Regional Key.

Description: Potentially hazardous food refrigerated

Noncompliance Area: During the renewal inspection on 8/12/25 plated food was observed sitting out on the kitchen counter in toddler classroom (2) hours before meal time. Food was observed during the hours of 10am-12pm.

Correction Required: Potentially hazardous food brought from the child's home or provided by the facility shall be refrigerated.

Noncompliance Area: During the renewal inspection on 8/12/25 bottles without children's names were observed in the downstairs refrigerator.

Correction Required: Disposable nursers and bottles shall be labeled with the child's name.

Description: General Health and Safety

Noncompliance Area: During the renewal inspection on 8/12/25 a ladder was observed on landing in the stairway to upstairs classrooms. Ladder was accessible to children and could pose a safety concern if the ladder was to fall on a child walking up or down the stairs.

Correction Required: Conditions at the facility may not pose a threat to the health or safety of the children.

Description: Comply with CPSL/CPSL information

Noncompliance Area: During the renewal inspection on 8/12/25 the file reviewed for facility person #1 hired (see code sheet), facility person #2 hired (see code sheet), facility person #3 hired (see code sheet), facility person #4 hired (see code sheet), and facility person #5 hired (see code sheet) contained a NSOR certificate more than 60 months old. The file reviewed for Facility #1, facility person #2, facility person #3 and facility person #4 also contained a mandated reporter certificate more than 60 months old. NSOR certificates were dated 02/26/20 and 07/01/20. Mandated reported certificates were dated 10/24/19.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Facility Person #1, #2, #3, #4 and #5 may not work in a child care position at the facility.

Noncompliance Area: During the renewal inspection on 8/12/25 the file reviewed for facility person #5 (who was identified as the director) did not include an updated NSOR certificate The NSOR certificate in the file was dated 02/26/20, which is more than 60-months old. Therefore, a director is not employed at the facility.

Correction Required: A director shall be employed by a facility and be present at the facility site a minimum of 30 hours per week.

Noncompliance Area: During the renewal inspection on 8/12/25 the older toddler classroom was observed to be out of ratio. There were 7 children present with staff person #4 and the youngest child in the group was 2 years old.

Correction Required: When children are grouped in similar age levels, the following maximum child group sizes and ratios of staff persons apply: Infants 1:4 with a maximum group size of 8; Young toddlers 1:5 with a maximum group size of 10; Older toddlers 1:6 with a maximum group size of 12; Preschool children 1:10 with a maximum group size of 20; Young school-age children 1:12 with a maximum group size of 24; Older school-age children 1:15 with a maximum group size of 30.
