Noncompliance Area: Staff #2 left the baby area to answer the door on 4/23/26 at approximately 9:35am, leaving the infant alone in a bouncy seat. Staff #1 could not visibly see the infant from where they were standing. The infant was unsupervised for about 2 minutes.

Correction Required: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises.

Correction Required: A written plan identifying the means of transporting a child to emergency care and staffing provisions in the event of an emergency shall be displayed conspicuously in every child care space and shall accompany a staff person who leaves on an excursion with children.

Correction Required: If disposable diapers are provided by a parent or by a facility, a soiled diaper shall be discarded by immediately placing the diaper into a plastic-lined, hands-free covered can.

Description: Age and training

Noncompliance Area: Staff #3 has not completed Pediatric first aid and CPR training on or before expiration of the most current certification. This is evidenced by the previously documented Pediatric first aid and CPR training on file expiring in February 2026. Documentation of updated Pediatric first aid and CPR training is not on file.

Correction Required: Competence is the completion of training by a professional in the field of first-aid and CPR. All staff persons shall renew their certification in pediatric first-aid and pediatric cardiopulmonary resuscitation (CPR) on or before the expiration of the most current certification. Until such time as the required training has been completed, staff person #3 must be supervised, when interacting with children at a minimum by, a primary staff person, who has completed all preservice trainings and has all qualifications to care for children unsupervised. If there are no staff person(s) available to supervise staff person #3, staff person #3 may not work in a child-care position at the facility.

Description: Supervise secondary staff

Noncompliance Area: On 4/23/26 at approximately 9:35am, in the main room, Staff #2 and Staff #1 were supervising 6 children. Staff #2, a primary staff, left the main room to answer the door. Staff #1, a secondary staff, was left unsupervised in the main room with 6 children for 1-2 minutes. On 4/23/26 at approximately 12:15pm, Staff #1, a secondary staff person, was caring for children unsupervised for approximately 5-7 minutes while Staff #2, a primary staff, went to get a child from the bus.

Correction Required: A primary staff person shall supervise the performance of a secondary staff person.

Description: Primary staff person present

Noncompliance Area: On 4/23/26 at approximately 12:15pm, Staff #1, a secondary staff person, was caring for children unsupervised for approximately 5-7 minutes while Staff #2, a primary staff, went to get a child from the bus.

Correction Required: A primary staff person shall be present in a facility when six or fewer children are in care, unless the staff-child ratio specified in §3280.52(b) or (c) requires a second or third staff person.

Description: Mixed age levels

Noncompliance Area: On 4/23/26 at approximately 9:35am, Staff #2, a primary staff person left to answer the door, leaving Staff #1, a secondary staff person, caring for 6 children (1 infant; 1 YT; 1 OT; 3 PS) alone.

Correction Required: When children are grouped in mixed age levels, the age of the youngest child in the group determines the staff:child ratio and maximum group size in accordance with the requirements at §3280.52(b).

Description: Building Surfaces

Noncompliance Area: Observed an exposed nail and splintering wood on the inside door frame of the gross motor space. Observed a damaged wall with a hole and crack in it above the sink in the bathroom on the far right.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.

Regulation: 3280.77(a)

Description: No peeling paint or plaster

Noncompliance Area: Peeling paint observed on the support post in the main child care room/space.

Correction Required: Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility.

Regulation: 3280.26(c)

Description: Training regarding plan

Noncompliance Area: The file for Staff person #2, a primary staff member, lacked documentation of emergency plan training.

Correction Required: The operator shall assure that each facility person receives training regarding the emergency plan at the time of initial employment, on an annual basis and at the time of each plan update. The operator shall document the date of each training and the names of all facility persons who received the training.

A facility person's record shall include verification of child care experience, education and training prior to service at the facility.

Noncompliance Area: The facility lacked a designated play space to be used for large muscle activity including running, jumping, climbing, and riding.

Correction Required: A facility shall provide outdoor or indoor play space to be used for large muscle activity which includes running, jumping, climbing and riding.
