Noncompliance Area: Facility self-reported non-compliance: Staff 1 and 2 lined up children at the classroom door to go outside. The staff did count the children then proceeded to the outside door (which was next to their classroom door, at the end of the hallway - less than a few feet away from the classroom door). The staff opened the outside door - one staff holding the door just out of sight of the classroom, the other at the front of the line- as the children exited, stopping at the gate to the playground, to do name to face when they realized a child was not with them. The staff backed up the look in the classroom and saw the child. The child was in the classroom, unsupervised, for less than 1 minute.

Correction Required: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises.

The two staff were retrained on the center's Supervision Policy and Threshold Counts and Name to Face policy. They had to watch a video on: The Threshold Count Name to Face Process and QA Supervision Policy Webinar and had to sign off that they watched it. Noncompliance Area: This complaint is verified due to Staff #1 and Staff #2 both stated to certification rep the infant was left alone in the classroom unsupervised during the fire drill.

Correction Required: Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises.

Each staff person reviewed and signed the "name to face" training. Each staff person completed an additional two trainings on supervision.
