Although the classroom contained the correct number of mats and cribs based on enrollment, the teachers failed to accurately reflect the appropriate number of beds in relation to the number of children enrolled in the infant classroom on the grid. The child was visiting another classroom at the time of the state inspection, and the teacher inadvertently failed to transfer the emergency contact information to the visiting classroom. As soon as it was identified that child #1's emergency contact information was not present in the classroom, it was immediately retrieved from the Infant #1 classroom and placed in the Infant #2 classroom bag to ensure compliance.

Immediately following the inspection, a conversation was held with the teachers within the classroom regarding appropriate classroom hygiene practices, including ensuring that teachers and students wash their hands before and after meals/snacks and after diapering/toileting. Staff were also reminded that hands must be washed after each diaper change prior to reapplying gloves for the next diaper change. Additional discussions will take place during our upcoming staff meeting, through one-on-one conversations, and during ad-hoc classroom visits to reinforce expectations and ensure ongoing compliance.

A follow-up conversation was conducted with the classroom staff, and an appropriate alternative storage location was identified. Moving forward, the bins will be stored underneath the cubby area while teachers continue educating the children on independently removing and returning their nap items at the conclusion of nap time.

The staff member's job responsibilities preclude her from being counted in classroom ratio. The water safety training was mistaken for the CPR certification. The staff member is scheduled to attend CPR training on Thursday, April 16, and has already completed the online portion of the course. She is not required to be in a classroom; however, if necessary, she will only be present under appropriate supervision. She will not be in a classroom independently until she has successfully completed the in-person CPR training on Thursday, April 16, at which point she will furnish to the state.

Correction Required: Tiered IS: 1. A facility person may not restrain a child by using bonds, ties or straps to restrict a child's movement or by enclosing the child in a confined space, closet or locked room. The prohibition against restraining a child does not apply to the use of adaptive equipment prescribed for a child with special needs. This portion of the plan shall have an immediate correction date. 2.The legal entity must arrange for all facility staff to receive a minimum of 2 hours of PQAS training regarding Behavior Management. The legal shall receive DHS approval of the training content prior to the scheduling of the training. The operator shall provide a date for when this training will be completed. 3. The legal entity must submit a policy to address managing challenging children with difficult behaviors. The legal entity shall submit their policy to the Regional Office for approval. Once approved, the legal entity shall review the policy with each staff person and have them sign off on the policy. This documentation shall be retained in each staff's file. The operator shall provide a date by when this will be completed.

Immediately after the state inspection concluded, we spoke with staff person #1 regarding the complaints received on February 24, which was founded. No staff will restrain any child. Date of correction: 3/18/25 Our Behavior policy has been updated to indicate appropriate measures of supervision that are to take place to help guide, correct and socialize children toward appropriate behaviors. We will review the updated "Teacher's Expectation: Child and classroom Guidance and Policy" with all staff members in our upcoming staff meeting on Thursday, March 20. Upon receiving the updated policy, all staff members will be required to sign and date an acknowledgment from indicating that they have received, read and understand the policy.
