Corrective Action: Ensure all school age children wear a face covering while inside the child care except for meals and naps. This requirement was discussed and corrected when staff agreed to comply in the future.

Corrective Action: Ensure staff-to-child ratios and maximum group size are maintained at all times. Upon arrival the assistant teacher was outside of the center. When the specialists entered the building, the assistant teacher did too. The assistant teacher joined an intern in the Pre-K room. There were 11 children ages three months to four years present. This was corrected when the specialist arranged the staff and children into proper staff-to-child ratios.

Corrective Action: Ensure safe sleep practices are followed. An infant was observed asleep in a bouncer, wearing a bib. This was corrected when the infant woke up.

Corrective Action: Room capacity must be maintained at all times. 11 children were observed in the Pre-K room and the capacity is six children. This was discussed and a plan was put in place to maintain capacity after nap.

Corrective Action: Ensure infants are not in confining equipment for more than 30 minutes. Two infants were observed in a bouncer and a bassinet for more than 30 minutes. This was discussed and corrected at the visit.

Corrective Action: According to attendance records submitted as a corrective action, an intern was alone in the Jr. Toddler classroom from 12:30 to 4:30 pm on 2/24/2020, from 3:00 to 4:36 pm on 2/25/2020, from 3:00 to 4:37 pm on 2/26/2020 and from 3:00 to 4:15 pm on 2/28/2020. This was corrected at the visit when the center was in ratio.

Corrective Action: Ensure room capacity is maintained at all times. There were seven children in the Pre-K room that measures for six. This was corrected when one children went into the main room with the owner.

Corrective Action: Ensure staff attendance accurately reflects where a staff is physically present. An assistant teacher was present in the Jr. Toddler classroom and signed in at 6:35 am. She was also signed into the Pre-K classroom from 6:35 to 9:00 am. This was corrected at the visit.

Corrective Action: Ensure unqualified staff are not alone with the children. An intern was alone in the Jr. Toddler classroom with four children ages infant and one. The intern was the only staff signed in on the attendance records. This was corrected when an assistant teacher entered the classroom.

Corrective Action: Ensure room capacity is maintained at all times. There were eight children in the Pre-K room that measures for six children. Submit one week of attendance records.

Corrective Action: Painting projects may not affect the air quality in the center. The center smelled like paint and a staff stated she painted today. The painting is now complete, so this was corrected at the visit.

Corrective Action: Ensure unqualified staff are not alone with children. An aide met the school age children at the bus stop. An intern was alone in the Jr. Toddler room with two children ages infant and one. Submit one week of attendance records.

Corrective Action: According to attendance records, the Sr. Toddler and Preschool classroom was over capacity by one child on 2/6/2020. The school-age classroom was over capacity by two children on 2/4/2020, five children on 2/5/2020 and three children on 2/6/2020.
