| Findings: During a review of children's emergency forms, it was found that six out of the eleven children who are enrolled, are missing yearly updates on their emergency forms. It was also found that a five year old boy is missing his physician's address and phone number on his emergency form. Additionally, another child is missing his 24 month old blood lead test. Children under the age of six years old must have a blood lead test at 12 months and 24 months.
