Staff did not supervise children to ensure safety and guide behavior. Licensing Specialist reviewed video footage of a child falling off a table on 3/12/25. The footage showed that if staff had intervened quicker, this could have prevented another child from climbing up on the table and pushing the other child off. The child pushed off the table did have a head injury. Violation
