Noncompliance Area: During the Renewal Inspection on 11/19/2025, The fire drill/fire detection log was up to date. However, the log showed a previous lapse in the requirement to conduct fire drills at least once every 60 days. The log showed lapses as follows: Fire drill was held 3/19/2025 then next on 5/21/2025, then next on 7/21/2025.

Correction Required: Fire drills are conducted at least once every 60 days.

Noncompliance Area: During the Renewal Inspection on 11/19/2025, The fire drill/fire detection log was up to date. However, the log showed previous lapses in the requirement to manually test the fire alarm system at least once every 30 days. The log showed lapses as follows: Alarm was tested on 1/10/2025 then next on 2/10/2025, tested on 3/7/2025 then next on 4/7/2025, tested on 7/4/2025 then next on 8/4/2025, tested on 8/4/2025 then next on 9/4/2025 and tested on 10/3/2025 then next on 11/3/2025.

Correction Required: Fire detection devices or systems must be in compliance with standards established under section 1016(c) of the act (62 P.S. § 1016(c)). The operator or designated staff person who is responsible for compliance with this chapter shall ensure the requirements in subsection (a) are met. The fire alarm system must be manually tested and documented at least once every 30 days.

Noncompliance Area: A renewal inspection occurred at the facility. A review of children's files was conducted. Regulation requires children's emergency contact and agreement forms to be updated at least every 6-months. The following forms for children' lapsed being updated every 6-months: Child # 1 had an emergency contact and agreement form dated for 3/13/23. The agreement for Child # 1 was updated on 12/12/23 but late. The emergency contact for Child # 1 has not been updated. Child # 3 had an emergency contact and agreement form dated for 4/20/23. The agreement form for Child # 3 was updated on 12/13/23 but late. The emergency contact for Child # 3 has not been updated. Child # 5 had an emergency contact and agreement form dated for 4/15/23. The agreement form for Child # 5 was updated on 12/12/23 but late. The emergency contact for Child # 5 has not been updated.

Correction Required: The parent shall update in writing emergency contact information once in a 6-month period or as soon as there is a change in the information. A parent is required to review and update the emergency contact information and the financial agreement at least once in a 6-month period or as soon as there is a change in the information.
