Correction Required: A written plan of daily activities and routines, including a time for free play shall be established for each group. The plan shall be flexible to accommodate the needs of individual children and the dynamics of the group. The written plan of daily activities shall be posted in the group space.

Noncompliance Area: In review of children's files, child 1's emergency contact and fee agreement failed to update within six months. Child's 2 fee agreement failed to be updated within six months.

Correction Required: The parent shall update in writing emergency contact information once in a 6-month period or as soon as there is a change in the information. A parent is required to review and update the emergency contact information and the financial agreement at least once in a 6-month period or as soon as there is a change in the information.

Noncompliance Area: In review of children's files, child 1 failed to have an updated health report contained in file. The last documented health report was completed on 02/21/25.

Correction Required: The operator shall require the parent to provide an updated health report at least every 6 months for an infant or young toddler. A child's record shall contain initial and subsequent health reports.

Noncompliance Area: In review of staff files, staff person 3 file failed to include a staff health report.

Correction Required: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem. A facility person's record shall include a written report of initial and subsequent health assessments, including the results of initial and subsequent tuberculin skin tests, x-rays or other medical documentation necessary to confirm freedom from communicable tuberculosis.

Noncompliance Area: In review of staff files, staff person 3 file failed to include TB screening by the Mantoux method.

Correction Required: An adult health assessment must include tuberculosis screening by the Mantoux method at initial employment. Subsequent tuberculosis screening is not required unless directed by a physician, physician's assistant, CRNP, the Department of Health or a local health department. A facility person's record shall include a written report of initial and subsequent health assessments, including the results of initial and subsequent tuberculin skin tests, x-rays or other medical documentation necessary to confirm freedom from communicable tuberculosis.

Noncompliance Area: In review of staff files, staff person 5's file failed to include college transcripts. File only included copy of degree which did not indicate area of studies.

Correction Required: A facility person's record shall include acceptable verification of experience, education or training is a transcript or a diploma or a letter signed by a representative of the experiential, educational or training entity.

Description: Health and Safety Training - Pediatric First Aid and CPR

Noncompliance Area: Staff persons 1 and 2 has not completed the following required pre- service training within 90 days of their date of hire (see LIS code sheet): Pediatric First Aid and CPR.

Correction Required: Staff persons shall complete professional development in the topic of pediatric first aid and pediatric cardiopulmonary resuscitation within 90 days of hire. Until such time as the required training has been completed, staff persons 1 and 2 must be supervised, when interacting with children at a minimum by, an AGS, who has completed all preservice trainings and has all qualifications to care for children unsupervised. If there are no staff person(s) available to supervise staff persons 1 and 2, staff persons 1 and 2 may not work in a child-care position at the facility.

Noncompliance Area: In review of staff files, staff person 1, employed more than 45 days, file failed to include child abuse clearance and out of state child abuse clearance and sex offender registry check. Staff person 2, employed more than 45 days, file failed to include NSOR certificate. Staff person 4, employed more than 45 days, file failed to include out of state child abuse clearance and sex offender registry check. Staff person 6, employed more than 45 days, file failed to include child abuse clearance.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). LACKING REQUIRED HIRING DOCUMENTS: Facility Persons 1, 2, 4 and 6 may not work in a child care position at the facility. A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL.

Correction Required: Protective receptacle covers shall be placed in electrical outlets accessible to children 5 years of age or younger.

Noncompliance Area: During renewal inspection on Wednesday, 02/11/26, cert rep observed hot water temperature in the girl's restroom exceeded 110° F. Temperature read at 120° F.

Correction Required: Hot water temperature, in areas accessible to children, may not exceed 110° F.

Noncompliance Area: During an unannounced inspection on Friday, 01/24/25, cert rep observed a preschool age child in room U8 sleeping on the floor with only a blanket. This is continued noncompliance from the renewal inspection on Tuesday, 12/17/24.

Correction Required: Individual, clean, age-appropriate rest equipment shall be provided for preschool, toddler and infant children as agreed between the child's parent and the operator. The rest equipment shall be labeled for the use of a specific child and used only by the specified child. Bed linens may not be used alone as age-appropriate rest equipment.

Noncompliance Area: During an unannounced inspection on Friday, 01/24/25, cert rep observed an aide in room 3 and room U8 working unsupervised.

Correction Required: An aide or a combination of aides shall be supervised at all times by a staff person qualified at minimum as an assistant group supervisor.

Noncompliance Area: During an unannounced inspection on Friday, 01/24/25, cert rep was informed that preschool age children was still receiving care in a space that is not measured childcare space included in the facility's capacity. This is continued noncompliance from the renewal inspection on Tuesday, 12/17/24.

Correction Required: Measurement and use of inodoor child care space. Indoor child care space is measured within permanent stationary partitions or walls. The allowable number of children in a space is determined by dividing the total square feet in a space by 40.

Regulation: 3270.106(a)

Description: Clean, age appropriate

Noncompliance Area: During renewal inspection on Tuesday, 12/17/24, cert rep observed several children in room 3 using rest equipment not labeled with their name. Staff persons stated that children who receive part time care share rest equipment.

Correction Required: Individual, clean, age-appropriate rest equipment shall be provided for preschool, toddler and infant children as agreed between the child's parent and the operator. The rest equipment shall be labeled for the use of a specific child and used only by the specified child.
