Noncompliance Area: Observed 9/12/2023 The file for staff # 1 and 2 does not contain a completed DHS required FBI and NSOR clearance. The requirement to have the completed DHS required clearances on file was not met. Staff #1 &2 passed their provisional hire dates.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Facility Person #1 and 2 may not work in a child care position at the facility.

Staff File1 Correction completed: The fingerprinting certificate was misplaced. Employee got a duplicate (electronic file sent). The copy indicated that we received their certificate during their provisional hire period. Staff File#2 Correction completed: This file had a fingerprinting certificate for the department of education which for some reason is not applicable for DHS. Employee immediately redid their clearances under DHS (Certificate sent). This employee is currently on administrative duty and not working directly with children until we receive their NSOR. This employee submitted four applications and called two times to get her clearance and we still have not received a response from the NSOR agency.
