Noncompliance Area: The file of child #1 had an initial health assessment more than 60 days from their first day of attendance at the facility.

Correction Required: The operator shall require the parent of an enrolled child, including a child, a foster child and a relative of an operator or a facility person, to provide an initial health report no later than 60 days following the first day of attendance at the facility. A child's record shall contain initial and subsequent health reports.

Noncompliance Area: Staff person #4 has completed mandated reporter training within 90 days of their date of hire (see LIS code sheet), however the mandated reporter training is not from an approved source.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). Until such time as the required training has been completed, staff person #4 must be supervised, when interacting with children at a minimum by an AGS who has completed all preservice trainings and has all qualifications to care for children unsupervised. If there are no staff person(s) available to supervise staff #4, staff #4 may not work in a childcare position.

Noncompliance Area: The file of staff #2 had Mandated reporter training completed more than 60 months apart due to the fact they were dated: staff #2 10-25-20 and 1-15-26.

PROVISIONAL HIRE UNSUPERVISED: A provisional employee may not be permitted to work alone with children and must work within the vicinity of a permanent employee. Noncompliance Area: The file of staff #1 had NSOR clearances more than 60 months apart due to the fact they were dated: 12-2-25 and 11-2-20. The file of staff #3 lacked out of state clearances and there was documentation they lived out of state in the last 5 years. The file of staff #3 had a late NSOR due to the fact it was dated 12-10-25 and they started in care with children on 12-8-25.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). LACKING REQUIRED HIRING DOCUMENTS: Facility Person #3 may not work in a child care position at the facility. Correction Required: A group supervisor shall have a bachelor's degree from an accredited college or university in early childhood education, child development, special education, elementary education or the human services field.

Noncompliance Area: Observed a sink in the older toddler/preschool room that lacked a handwashing sign.

Correction Required: A facility person and an able child shall wash his hands after toileting and before eating. A sign on which this requirement is written shall be posted at each toilet, training chair, diapering area and sink in the facility.

Noncompliance Area: The file of staff #1 lacked an acceptable initial health assessment due to the fact it did not assess free from communicable disease the person's suitability to provide child care.

Correction Required: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem. An adult health assessment must include the physician's or CRNP's assessment of the person's suitability to provide child care.

Description: Supervised at all times

Noncompliance Area: Observed at 1:10 p.m., during naptime in the toddler room, child #1 located in the far end of the dining space and staff #1 sitting on the opposite far end of the space. There were wooden shelving units, tables, and chairs occupying the space between child #1 and staff #1. Therefore, staff #1 could not effectively supervise child #1.

Correction Required: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. Children must be supervised at all times.

Regulation: 3270.124(e)

Description: Written emergency plan posted

Noncompliance Area: At the time of the inspection, the means of transporting a child to emergency care and staffing provisions in the event of an emergency was not posted in the infant room.

Correction Required: A written plan identifying the means of transporting a child to emergency care and staffing provisions in the event of an emergency shall be displayed conspicuously in every child care space and shall accompany a staff person who leaves on an excursion with children.

Noncompliance Area: Staff person #5 has completed the health and safety course titled Getting Started with Center-Based Care: Building Blocks for Quality (10 hours) but has not completed the required health and safety update course titled Pennsylvania Health and Safety Update 2022 (1 hour) training.

Correction Required: A facility shall be operated in conformity with applicable Federal and State laws and regulations. State agencies whose regulations may relate to the operation of a facility include the Department of Environmental Resources, the Department of Labor and Industry, the Department of Health, the Department of Education and the Department of Transportation. Conditions at the facility may not pose a threat to the health or safety of the children. Staff person #5 must complete (PA Required Health and Safety for Centers-Group Child Care Parts 1 and 2/ PA Required Health and Safety for Family Child Care Parts 1 and 2).
